    Medical Form

    All prescribed medication must be brought to the main school office by a parent or guardian, where a medication form will need to be completed with all relevant information. A copy of this form is also available at the bottom of this page for convenience. Please note that the school is unable to administer any non-prescribed medication—including items such as Calpol, pain relief, or travel sickness tablets. If a child requires these, a parent or guardian will need to attend school at the appropriate time to administer the medication.

    To help us keep your child safe and supported throughout the school day, please let the school office know about any changes to your child’s current medication. It’s also important that we are made aware of any medical conditions that may affect them during school hours, as well as all allergies—no matter how mild they may seem. Keeping this information up-to-date helps us ensure your child receives the care they need while they are with us.

    Change of Details

    To help us keep your child’s records accurate and up to date, any changes must be submitted in writing to the school office. This includes updates to your address, contact numbers, or email address. A form is attached below for your convenience and can be completed and returned to ensure we have the correct information on file. Keeping these details current helps us stay in touch and ensures we can contact you quickly if needed.

    Leave in Term Time

    Please see the Leave in Term Time form below. We would like to remind families that, following guidance from Birmingham City Council, leave during term time cannot be authorised unless there are exceptional circumstances. We appreciate your understanding and support in helping us maintain good attendance for all children.
